To Order: Just fill out this form, print it, and mail it to us.   Music is available in Cassette or Compact Disc ONLY.  Cassettes are $10.00, CD's are $15.00 each, plus $1.50 each postage and handling for either option.  Please make checks payable to: "us".  Allow 2-3 weeks for delivery.
